Renewals are in the following January of each year, whether you apply in the first or second enrollment

A HARTIST IS AN INDIVIDUAL WITH AN EMOTIONAL OR PHYSICAL DISABILITY* AND FINANCIAL CHALLENGES** WHO USES THE CREATIVE ARTS TO ENERGIZE AND IMPROVE THEIR OVERALL WELL-BEING. IT IS NOT THE END RESULT OF THE ART PROJECT THAT IS THE GOAL. IT IS THE PROCESS OF CREATING ART, ACCORDING TO SCIENTIFIC RESEARCH.

If approved, a Hartist receives quarterly a box (or bag by choice) of random art supplies, which include (but may not be limited to) 1) Jewelry Making 2) Mixed Media 3) Paper Crafting 4) Junk Journal / Booksmithing 5) Fabric/Yarn/Fiber Arts 6) Many other crafts that fall under creative arts.

IF YOU’RE INTERESTED IN BECOMING A HARTIST, PLEASE ANSWER THE FOLLOWING QUESTIONS TO THE BEST OF YOUR ABILITY. MUST BE OVER 18 YEARS OLD TO APPLY. LIMIT ONE HARTIST PER HOUSEHOLD.

Please note: When submitting your financial documents as proof of low income, we do not accept your Social Security Disability Statement or your Social Security Benefit Letter as proof of income. This is because even millionaires can receive these, but your Social Security Disability statement is one of the accepted forms of disability proof.
Only those who are disabled and in a tough financial position can become a Hartist. We approve at 150% of the poverty level.….

You ONLY need ONE of the below to qualify as PROOF YOU ARE DISABLED
(1) SSD Benefit Verification Letter -

(2) SSi statement of benefits IF under 65
(3) If over 65, then disability statement from your primary care doctor

You ONLY need ONE of the below to QUALIFY FINANCIALLY

**Your Uploaded files must be 10mb or smaller EACH…. PDFs are usually large and rejected… please make sure every line is filled out, uploaded docs include all pages and are 10mb or smaller and multiple choices completed.

  • Supplemental Security Income Statement for this year (this actually covers both your financial proof and proof of disability)

  • Food Stamps/EBT benefit letter for this year - A food stamps/EBT benefit Letter is only accepted with an expiration date beyond Feb 15 of this year (contact your local Department of Social Services or the agency that manages SNAP in your area)

  • The IRS Tax return you filed this year (for last year) that shows you at or below poverty on gross income (Include both pages and your proof of submission)

  • Medicaid Benefit Letter for this year

    The above are the ONLY forms accepted for proof of financial status.
